 Dr. Evangelista is an independent healthcare provider with contracts with many third party payers including HMP, PPO and indemnity insurance networks. We are also Medicare providers. The following is a list of insurance plans we accept. Please keep in mind that contract negotiations are continuous and new plans are added frequently. If your present plan does not appear on our list, please contact our office at (202) 331-1234. Our office staff would be happy to check with your insurance company to determine any out of network benefits to which you may be entitled. Hospital charges are covered by most insurance companies even if we are not participating providers.
